Don't Make Us Call Your Mom!



By checking the box and singing your name, and in consideration of being permitted to voluntarily compete, officiate, observe, and/or participate in the Great Lakes Sport & Social Club (â??SSCâ??) events (hereafter, jointly and inclusively, the â??Eventâ??), you hereby acknowledge that you have read and agree to the following Waiver and Release of Liability:

This waiver, by extension, covers and applies to the Great Lakes Sports and Social Club, its chapter cities including, but not limited to Grand Rapids Sport & Social Club, Kalamazoo Sport & Social Club, any additional subsidiaries, assumed names, affiliates, employees, agents, assigns, independent contractors, sponsors, and partners.

Waiver & Release of Liability

I acknowledge that there are certain risks of physical injury (whether minimal, serious, or resulting in death) as well as damage to personal property as I participate in the Event. Each Event has foreseeable risk of injury that may result from my voluntary participation in the Event or as a general hazard of my participation. Iâ??m aware of the hazards of the Event and I agree to voluntarily participate. I also agree to waive any liability that may arise associated with my participation in the Event. As such, I hereby agree to the following:

  1. I hereby release, indemnify, and hold harmless Company, its officers, directors, employees, agents, volunteers, and contractors (collectively, â??Releaseesâ??) from any claim, demand, loss, liability, damages, and attorney fees and costs whatsoever arising from, related to, or resulting from these Risks (â??Claimsâ??), including those caused by the negligent acts or omissions of any or all of the Releasees.
  2. I recognize the physical exertion involved in the event and attest and certify that I am physically fit to compete safely, and I have not been advised otherwise by a health care professional.
  3. As between each of the Releasees and me, I will be solely responsible for any and all medical and related bills that I may incur because of any injury, as well as costs related to loss or damage to my property, that I may sustain as a result of my participation in the Event, including those sustained on the premises where the Event is conducted and while I am traveling to and from such premises, regardless of the location or mode of transportation.
  4. This Agreement shall be binding on my estate, heirs, executors, administrators, successors, and assigns, as well as any other party asserting a Claim on my behalf or on behalf of my estate.


General Provisions


  1. I hereby expressly agree that (1) this Agreement shall be governed and construed according to the laws of the state of Michigan without regard to its conflict of laws provisions and (2) any action or proceeding concerning any Claim or the meaning or effect of any provision of the Agreement shall be conducted only in the federal or state courts located in Grand Rapids, Michigan, and that for such purposes, I expressly submit to the jurisdiction of such courts.
  2. This Agreement contains the entire understanding between and among the parties concerning these matters. No waiver, modification, or amendment of any of the terms of this Agreement shall be effective unless made in writing and signed by the party to be charged.
  3. I hereby expressly agree that if any portion of this Agreement is held invalid, the balance of the Agreement shall nonetheless continue in full legal force and effect.


Player Safety & Privacy Disclaimer


  1. Please note that all the sports and events we play here at SSC have risks associated with them and you are fully responsible for looking after yourself. Also, we ask that you look after each other. If you or any other teammate is not comfortable, feels unsafe, or is at risk of injury, you have a duty to reach out to a SSC team member who will address the situation. By participating in SSC sports or activities, you acknowledge that you have read and agree to the SSC Waiver & Release of Liability, Assumption of Risk, and Indemnity Agreement above. These sporting activities are social and are not led by a qualified instructor. Please consult your doctor if youâ??re unsure whether youâ??re fit enough to be taking part in these sports, as even a social beginner sportâ??s game can be physically demanding.
  2. Please be aware that you are responsible for warming up before playing any sport, cooling down, and stretching. Those participants wearing glasses are advised to consider wearing contact lenses, and those wearing any jewelry or loose accessories (including but not limited to watches, earrings, necklaces, other valuables) MUST remove them before playing. Fingernails MUST be trimmed short at all times to avoid risk of other injury to other participants. In wet weather, please bring an extra set of shoes and take care while running on the courts to avoid slipping. The only permitted footwear is non-marking sports shoes if participating at an indoor facility. You are also responsible for ensuring that your shoes are appropriate for the sport or activity that you are participating in. Please turn off your cell phones and store them in your car or other secure location prior to the beginning of the sport or activity. You are solely responsible for any personal affects you bring into the athletic facility where your sport or activity is taking place.
  3. All Events are to be friendly and not intended to be taken seriously, so we encourage teamwork, a social atmosphere, and friendly competition. Any aggressive behavior will not be tolerated. If youâ??re looking to for a competitive league to let out stress and aggression, SSC is not for you. Player-directed profanity, fighting-words, and/or derogatory name calling is not tolerated under any circumstances. Furthermore, physical confrontations and altercations are strictly prohibited and may result in immediate suspension or termination without refund. We encourage a fun and social atmosphere and will protect our playersâ?? ability to enjoy themselves while participating in SSC events. All of our players must be able to play their sport or activity in safety. Spectators are welcome but their behavior is your responsibility and any adverse behavior as described above may result in immediate suspension or termination of your membership without refund.
  4. No smoking is permitted at any of the sports facilities or on their grounds. No alcohol or drugs are permitted at any of the sports facilities or their grounds. Additionally, no eating is allowed in any of the indoor sports facilities. SSC takes no responsibility for your valuables you choose to bring in the facilities, either from theft, loss, or from damage.
  5. By signing up to participate with SSC and its events, you acknowledge and represent that you 21 years of age or older. While SSC is a sport and social club, it does not encourage the irresponsible use of alcohol. Furthermore, SSC does not condone the choices of any participants to drink and drive. SSC, itâ??s partnered bar, and its sponsors are not responsible for your actions and any damages that may result thereof. If you are irresponsible and drink and drive, you do it at your own risk. SSC encourages you to refrain from breaking the law and instead, call a cab, walk, or designate a sober driver for the night. If you are concerned about finding your way home, please discuss with a SSC team member.
  6. SSC, its partners, and sponsors will not be held responsible for any incidental and/or consequential damages incurred from your participation. You play and socialize at your own risk.
  7. It is understood that your participation means that you have read, understood, agree, and will abide by these Rules, and the Liability Waiver, Assumption of Risk, and Indemnity Agreement as detailed above.

  9. We are an Adult League for those participants 21 years of age and older, so we strongly advise NOT to bring anyone under the age of 21 to any SSC Event, even as spectators. SSC players and team members do not want to act as a babysitter to young children. Furthermore, SSC harbors a strong concern for mixing minors with adults, especially when alcohol is involved. If you do bring a minor or individual under the age of 21 to a SSC Event, you do so at your own risk and assume any liability associated with their presence. If you notice a minor present at an SSC Event, please notify a SSC team member immediately.


Registration Disclaimer


SSCâ??s registration is first come, first serve; meaning, when we hit our max number of players registered, the registration will close automatically. If this happens when your team is still considered partial under SSC policy, you will be merged and/or moved to/with another team, at the sole discretion of SSC. Partial, â??non-securedâ?? teams also are subject to having Free Agents placed on it. SSC will be as fair as possible but once the registration is close, no new players will be added, no paid players can be moved to another team, and refunds will not be issued. So, the bottom line is: be sure to get all your friends signed up and paid before itâ??s too late. We recommend you consistently check your team roster to ensure everyone you want is on your team (itâ??s your captainâ??s fault if you leave a friend behind!) and that you made it onto the right team. Also, captains, be sure to secure your team with the right amount of players, otherwise SSC will do the filling!
